Broadway Kids Academy is a privately owned and maintains the right to refuse service to anyone.

By clicking "I Agree", I and my child agree to comply with all BKA rules, codes of conduct, policies and procedures. I understand that mine and/or my child's failure to comply could result in the withdrawl of my child from the BKA programs. See policies and procedures.

Acting on behalf of my minor child, I do hereby waive and release "Broadway Kids Academy for the Performing Arts" and all it's respective officers teachers, assistants, employees, sub-contractors, and/or representatives, from any and all liability for personal injuries or damages sustained, incurred, or arising from participating in "Broadway Kids Academy for the Performing Arts".

I give my permission for my child to participate in all classes, performances, activities and or field trips when offered by BKA. I do hereby authorize the adults supervising the activity to obtain emergency treatment for my child if necessary and to act in my stead in approving necessary medical care. This authorization shall cover the activity and travel to and from the activity.

I give my permission for Broadway Kids to use any photographs and or videos of rehearsals, performances or class participation that may be taken of my child for advertisment, media and social media purposes. No personal information including names will be used by BKA unless pre-approved by parents.
